Home Nursing Care
Provides direct nursing and care coordination to clients of all ages with acute, chronic, palliative health care needs in their homes and other community settings -mobilization of community resources and teaching self care.
Home Health Nurses are part of the comprehensive Hospice Palliative Care service in Fraser Health which enables patients with an advanced serious illness to remain at home as long as they may desire. This service includes a response telephone call-in line in partnership with the BC Nurseline to ensure our palliative care clients and their families have the ability to contact a nurse 24 hrs a day 7 days a week.

Long Term Care
Provides case management services to eligible adults 19 years of age and over with on going health challenges, who require assistance to remain independent in their home environment. Facilitates admission to Long Term Care facilities, assesses clients in their homes for funded services, arrange home making services, refer to other resources as needed.

Community Care Liaison Program
Provides for timely discharges from PAH hospital with supports and follow up/referral as needed and the prevention of hospital admission or readmission. Provides services (usually home support) on a short-term allowing a convalescent period at home. If long term needs become apparent referrals are made to the appropriate agencies , assess clients in hospital for a Long Term Care facility, works with facilities to return clients from acute to facility, enhances home support hours for those known to Home Health ready to return home. The PAH liaisons provide liaison service for all Home Health programs.

Clinical Nurse Educators(CNE) – Fraser South
-Provides clinical support to the Home Nursing Care programs in the Fraser South. The CNE for wound care is a certified wound care specialist, provide research, education, consultation to the Home Nursing Care programs. Work closely with counterparts in acute and community across the FH

Health Services for the Community Living (HSCL) – Fraser South
Provides nursing and rehabilitation consultation and health care planning for adults who are mentally and physically challenged and are clients of Ministry for Children & Family Development. Health assessment, teaching and monitoring of caregivers, health advocacy, liaison and consultation between families, community agencies, and health professionals. Work very closely with Ministry for Children & Family Development.

Home Health provides funding for:
- White Rock/South Surrey Come Share Adult Day Centre
The Adult Day Centre provides therapeutic recreational and social supports to eligible seniors living in the community and respite for caregivers.
- Bath Program at WR Come Share Adult Day Centre – provides tub baths for eligible seniors living in the community.
- WR/SS Meals on Wheels
- Senior Support Services provides some congregate meal programs, telephone support, visits to isolated seniors, resource information etc.
